At Lakeview Women’s Health, we understand that your time is precious.
We strive to make your appointment run as smoothly and on time as
possible. In order to ensure timeliness, we have provided the forms you
will need to have read and filled out as a new patient to our practice.
Please download and print these out, complete and sign where necessary,
and fax or bring them to your first visit. Also, please remember to
bring your insurance card, a picture ID, and any copay you are required
to pay by your insurance plan. Every patient will need the following
